Each package bears a label with establishment number “EST. 474” inside the USDA mark of inspection, in addition to lot code “01351” and the best-by date as noted above. The products subject to recall were produced in December 2010 and were sent to distribution centers in Calif., Colo., Md., N.Y. and Pa. for further distribution to retail stores.

The products subject to recall in packaging for retailers include:
• Whole chub packages of “SELTZER’S BEEF LEBANON BOLOGNA” with lot codes “01351” or “06337” and best-by date of “Feb. 14 2011,” “Feb. 15 2011,” “Feb. 16 2011,” or “Feb. 28 2011” printed on the package.
• Half chub packages of “SELTZER’S BEEF LEBANON BOLOGNA” with lot codes “01351” or “06337” and best-by date of “Apr. 6 2011,” “Apr. 7 2011,” “Apr. 16 2011,” or “Apr. 19 2011,” printed on the package.
• 12-pound bulk boxes of sliced “SELTZER’S BEEF LEBANON BOLOGNA” with lot code “01351” and best-by date of “Apr. 21 2011” printed on the package.

The products subject to recall were produced in December 2010 and were sent to distribution centers in Calif., Colo., Md., N.Y. and Pa. for further distribution to retail stores.

FSIS was notified of an investigation of E. coliO157:H7 illnesses on March 10, 2011. Working in conjunction with the Centers for Disease Control and Prevention (CDC) and state public health partners, 4 case-patients in N.J., N.C., and Pa. have been identified with illness onset dates between Jan. 10, 2011 and Feb. 15, 2011.

As a result of the epidemiologic investigation, FSIS determined that there is a link between the Lebanon bologna products and the illnesses in these states. FSIS is continuing to work with the CDC, affected state public health partners, and the company on the investigation. Anyone with signs or symptoms of foodborne illness should contact a health care provider.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers.
E. coli O157:H7 is a potentially deadly bacterium that can cause bloody diarrhea, dehydration, and in the most severe cases, kidney failure. The very young, seniors and persons with weak immune systems are the most susceptible to foodborne illness.

The recall was issued after the company received a report that one 11-month-old girl became entangled in the drawstring. The child was freed by her mother, the CPSC said.

The recall affects 11 different models of BOB single and double strollers.

The strollers being recalled have a yellow or orange drawstring at the back of the canopy.

Fault involves wiring covers blowing off

In the recall notification, the CSPC notes that off-gassing from degraded inverter components, when combined with an electric arc, can create an explosion which can blow the cover off the wiring compartment. Xantrex parent company Schneider Electric has received five reports of wiring compartment covers blowing off, with no injuries or property damage reported.

The Xantrex GT inverters being recalled were manufactured in China and sold by solar distributors and system integrators in the United States and Canada from September 2005 through January 2011. The CPSC notes that now that the recall is in place, it is illegal to resell or attempt to resell these inverters.

Free repair of inverters offered by Xantrex

The CSPC states that Xantrex customers should immediately contact their authorized dealer to set up an appointment for free repair of the inverter. The CPSC also notes that it is still interested in receiving incident or injury reports that are either directly related to the product recall or involve a different hazard with Xantrex Grid Tie Inverters.

This recall has been initiated due to concerns expressed by the Food and Drug Administration (FDA) regarding the potential for the product to be inadequately sterilized. However, because the product is not being used during a sterile procedure, we do not believe it poses any risk to our customers.

This recall extends to all lots of Sterile Lubricating Jelly remaining within their labeled expiration dating (three years), including all lot numbers beginning with the digits 7, 8, 9 or 0. At least seven people, including four in Washington and three in Oregon, have been sickened with salmonellosis. Sprouters Northwest of Kent, WA, produced the sprouts identified as the source of the illnesses, according to Oregon health officials.

Sprouts, like bacteria, need warm, humid conditions to grow. They have been repeatedly identified as the cause of salmonellosis outbreaks, E. coli O157:H7 infections and other diseases, and have been responsible for at least 40 outbreaks of foodborne illness in the U.S. in the past 20 years. Jimmy John’s sandwiches have been associated with four sprout outbreaks in the last two years.

In December, the Centers for Disease Control and Prevention, the Food and Drug Administration and the state of Illinois announced that at least 97 people had been infected with Salmonella from contaminated alfalfa sprouts linked to an Illinois organic farm, Tiny Greens, that supplied sprouts to the Jimmy John’s sandwich restaurants in the Midwest.

But different sprouts from a different supplier, and a different strain of Salmonella, are responsible for the Pacific Northwest illnesses, said William E. Keene, senior epidemiologist at Oregon Public Health Division, in an email. “Just a coincidence that will keep happening as long as people keep eating this stuff, it would seem.”

The recalled sprouts from Washington state include Clover, Clover & Onion, Spicy Sprouts and Deli Sprouts varieties–all with the Sprouters Northwest brand name. They were sold in Oregon, Washington, Idaho, Alaska, and possibly other states, as well as in British Columbia. They are sold in retail stores, including QFC, Fred Meyer, Albertsons and Safeway, in 4-ounce and 5-ounce plastic containers, and in larger 1-pound bags and 2-pound trays to grocery stores and wholesale suppliers, which in turn supply the sprouts to restaurants and other outlets.

“This is at least the 13th sprout-caused outbreak that has sickened Oregonians since 1995, when we first started warning consumers about the risks of eating sprouts,” Keene said in a news release. Four of those outbreaks have involved Sprouters Northwest.

In this latest outbreak in Oregon, a Bend resident infected with Salmonella told her local health department she had eaten sprouts at a newly opened Jimmy John’s outlet. Then, after a 3-year-old boy in Bend also got sick, his parents said he had eaten a Jimmy John’s sandwich. The Deschutes County health authorities, mindful of the Midwest outbreak, alerted the state health department.

“It was the coincidence of the first case reporting that exposure that got the local health department interested in following up more than usual and that led them to involve our office in the inquiry,” Keene said. Oregon contacted the FDA and Washington state, which found four more cases of Salmonella Newport associated with Sprouters Northwest clover sprouts. On Monday, shortly after the recall was announced, a woman in Multnomah County, Oregon, was also confirmed as being infected with the outbreak strain.

Sprouters Northwest immediately recalled its clover sprouts once it heard about the outbreak, according to Keene. “They have been cooperating fully with the ongoing investigation,” he said.



State and federal agencies are working with Sprouters Northwest to remove potentially contaminated sprouts from distribution and to identify the source of the problem–which regulators say usually is contaminated seed. The company has suspended production while the investigation is underway.

Retailers and wholesalers that have any of the recalled sprouts should segregate them from other produce, Keene said. They can contact their distributor or Sprouters Northwest at 253-872-0577 for additional information.

Restaurant and deli operators should check their stock to identify and pull any of the recalled products, Keene advised. Consumers who have any of the recalled products should discard them.

Those three children are now out of the hospital, said Dr. Alan Melnick, Clark County health officer, on Friday afternoon.

Officials did not release any additional personal information about any of the four children.

However, the children did attend the same Vancouver day care provider, and seven other people who attended the site or worked there have tested positive for the bacteria. The center in Hazel Dell has been operated by Dianne and Larry Fletch for 20 years.

Melnick said the day care center was temporarily closed on April 2 while Clark County Public Health monitors the outbreak.

Twenty-two children attended the center, Melnick said.

The Fletches issued a statement late Friday afternoon expressing their sorrow:

“This is a very difficult time for the family who has suffered such an incredible loss.

“It is also a difficult time for our daycare families and the children who were his friends.

“It is an especially difficult time for us as daycare providers.

“We would like to tell the story of what happened, of how hard everyone worked to try and avert such a tragedy and how closely the public health department and our daycare licensing staff worked with us to identify the problem initially and to put into place measures to control the spread.

“However, at the moment, we are all grieving too deeply over our loss.”

While the day care center is a common element in the spread of the infection, investigators haven’t been able to identify the original source of the outbreak. It might remain a mystery, Melnick said: “In most cases, we never learn.”

The infection was spread from person to person, starting on about March 19.

When one meal or event is the source of an outbreak, people get sick at the same time, Melnick said.

Those tests identified E. coli O157:H7, which is the most common toxin-producing bacteria in North America, according to the federal Centers for Disease Control and Prevention.

The original source of an infection is often cattle manure, which can enter the body with undercooked hamburger or unpasteurized milk or fruit juices.

An infected person can transmit the infection to another person through human stool, when people don’t thoroughly wash their hands after using the toilet or after diapering a child.

Symptoms can take up to 10 days to appear. The symptoms often include severe stomach cramps, bloody diarrhea and vomiting, says the CDC’s Web site. Most people get better within a week. Some infections are very mild; but others are severe or even lead to a life-threatening complication known as hemolytic uremic syndrome, which is a major cause of acute kidney failure among children in the U.S.

People can pass along the disease before symptoms appear, so it is important to stop the transmission process.

“We believe we have it confined,” Melnick said.

Clark County Public Health staff will closely monitor children and day care employees for several more days. Any children or staff members who develop symptoms will not be allowed to attend or work in a day care facility until their symptoms resolve and they have two negative tests 24 hours apart.

The health department will continue to test staff and children and keep them away from the center until they are negative for the bacteria on two consecutive tests 24 hours apart.

“I don’t like closing a day care. Parents who are desperate will take their children someplace else and expose a new group,” Melnick said. “We are following staff and the children closely.

The affected families are having to change their child care plans, health officials said: maybe a parent stays home from work, or a relative watches the child.

Marni Storey, the county’s public health services manager, emphasized that those family caregivers must follow prevention steps and wash their hands thoroughly.

“We don’t them to get sick, too,” Storey said.

There have been at least three injuries to babies, including bumps, bruises and a head injury. Dorel Juvenile Group Inc., of Columbus, Ind., has received 77 reports of the child restraint handle fully or partially coming off the products.

The recall involves Safety 1st, Cosco, Eddie Bauer and Disney branded infant car seat carriers with certain model numbers. They were sold at department and juvenile product stores nationwide from January 2008 through this month.

The recall was announced on Friday by the Consumer Product Safety Commission, National Highway Traffic Safety Administration and Dorel Juvenile Group.
